To help us provide you with efficient service, the forms listed below will be sent to you by text or email after you have scheduled with the practice. You will be able to fill them out directly from that link and when you hit submit, they will come directly back to the office. There is no need to print them, fill them out, and bring them with you.
If you do not have texting or emailing capability, then you may print the forms below, fill them out, and bring them with you at your appointment. If you do not have a printer, please plan on arriving 15 minutes prior to your scheduled appointment to be able to fill them out in the office.

Appointments
Our practice will always respect and honor your appointment time. We strive to see you promptly so you can get on with your day. If you are unable to keep your scheduled appointment, we ask that you give us 2 business days’ notice so we can offer your time slot to another patient.
Insurance Information
We accept most major dental insurance plans. Costs not covered by your insurance provider are due at the time of treatment. We will work with you in determining the best possible payment options for you and help you maximize your insurance benefits.
Whitening For Life Program
We are proud to offer our patients the “Whitening for Life” program. The program, for a one-time $99 fee, is geared toward our patients who are diligent with their dental care. After an initial exam, cleaning, and x-rays, we will determine the type of cleaning you need based on your dental health.
We will then take impressions of your teeth in order to custom fit you with whitening trays. We will give you one 5 gram tube of whitening gel initially, and at six month intervals, we will replenish the whitening gel as long as you receive your exam and cleanings from us every six months.

Do you accept children?
Absolutely! Dr. Nickoles works wonderfully with children!
Do you offer sedation/nitrous?
We do offer Nitrous Oxide for those who wish to have it. Whether it be for restoration or just a cleaning, whatever makes you the most comfortable.
Do you have payment plans?
We have various payment plans such as, in-office monthly payments or CareCredit.
Do you take Medicaid?
We accept all patients, however, our Medicaid patients choose one of our other payment options, such as, pay out of pocket, CareCredit, or our in-house NIC Membership.
Do you accept my insurance?
We work with all insurances that offer out-of-network benefits. We will submit it on your behalf and the balance left after the insurance payment will be the patient’s responsibility. If you are unsure if your plan has out-of-network benefits, we can perform a complimentary benefits check when scheduled.
